New patients

The practice is open for new patients residing in the postal area 3581 to 3585 (for details see www.huisartsutrechtoost.nl and "Inschrijven online").

If you want to register as a new patient, please fill out the registration form Homeruslaan and we will mail you a return form in English.

You will need to let your former family practitioner know you are leaving the practice and bring your medical records with you or let them send it to us.

If you are visiting Utrecht (and are a non-resident) and you are in need of medical care, you can also contact the assistant to make an appointment. In that case you will have to pay for the consultation directly in cash(€50), and will receive a receipt which you can use for your insurance. Sorry, no ATM machine.
